How to Sign Consent Form

Please Login to the portal.
Navigate to the consent page tab.
After going to the consent details, you can provide signature.
Two types of signatures option available.

Digital signature
Physical signature

Digital signature: -

After navigating to the consent tab, at the bottom of the PAGE you can find the tick box, by clicking the tick box you will find the blank space to provide digital signature, which you can sign using your mouse.

Please create your digital signature. If you make a mistake, click on 'Clear signature’, and then create it again.

Physical signature: -

In case you are unable to provide digital signature you can also sign using physical signature where you have to download the consent form, mark the signature and upload the consent form, below are the steps to do the same.

At the top of the consent page, you can find the link “Physical Sign Design”, you can click on the link.
After clicking on "Physical Sign Design, “Download Authorization Report” will appear, click on “Download Authorization Report” then the form will be downloaded, then sign in the downloaded form and upload it by clicking on the button below “Attach Physical Signed Document.”

Step 10. Before submitting, please double-check all your details again. Once you've confirmed everything is correct, click on the “Submit” button.
